The transition from hospital to community settings is feasible, and there has been a decrease in observed disabilities. Social distance and stigma have significantly decreased in communities. Ethnographic observations highlight the importance of a sense of family that offers social support. There has been an improvement in reintegration rates and a reduction in the average number of re-admissions and inpatient days. Altruist has facilitated the de-institutionalization of more than 30 patients since 2022. The project aims to provide a systematic lifestyle with medical care to these individuals, integrating them into the community with self-respect and dignity.  This project receives financial support from Azim Premji Foundation and The Banyan.
